Under general supervision performs evaluations/assessments/home visits/chart reviews for a caseload of medically fragile children and youth. Assess and coordinate new referrals within established guidelines. Makes referrals and provide resources as needed. Maintain accurate and updated records. Completes authorization for services related to child’s chronic medical condition and develops and implements treatment plans for each child.
Master's degree in any behavioral science from an accredited college or university OR
Bachelor's degree in Social Work from an accredited college or university AND
One year of experience at lower level Social Services Specialist 1 (SSP070) or position equivalent.
In addition to the minimum qualifications, preference will be given to applicants who have experience with ICD-10 coding and verification, strong verbal and written communication skills. One year case management experience. Preference will be given to: Licensed Registered Nurse, Licensed Social Worker (LCSW), Licensed Master
Social Worker (LMSW), or a
Licensed Dietitian/Registered Dietitian (LD/RD).
