Under general supervision, performs in home comprehensive assessments of client’s health care needs within the guidelines/standard operating procedures of the Children 1st
/First Care Programs. Develops comprehensive plans of care and provides case management to patients/families consistent with the plan of care, program deliverables, and district policies and procedures. Provides developmental screenings and patient education and counseling. Advises caregivers, makes necessary referrals, and performs other duties as needed.
Bachelor's degree in Nursing from an accredited college or university AND
current Georgia license to practice as a Registered Professional Nurse AND
CPR certification OR
Associate's degree in nursing from an accredited college or university AND
One year of PHN experience AND
completion of a PHN course within one year of hire AND
Registered Professional Nurse license AND
In addition to the minimum qualifications, applicants who have experience working with diverse populations, including women, infants and children is desired.
