Case Manager


Summary

The City of Hinesville is accepting applications for a Case Manager with the Homeless Prevention Program.  This position is responsible for providing Case Management services to the Homeless Prevention Program. The Case Manager professional will work with program participants and/or families to obtain self-sufficiency, social services and financial guidance; provides client assessments; implement case management plans; monitors clients progress; conducting ongoing assessment for program eligibility/compliance; ensuring needs of clients are met; performs related duties as required.

Major duties: 

  • Work cooperatively and constructively with the Department of Community Development and Homeless Prevention Program staff, and with formal and informal systems to ensure an appropriate care plan that meets the needs of the participant;
  • Manage a caseload of 25-35 and complete home visits and phone contacts both as required and as needed to assess program eligibility and compliance, ongoing program eligibility and compliance, as well as ongoing care plan for identified/continued needs;
  • Schedule and/or participate in Case Conferences/Reviews with the participants, program coordinator and/or program director regarding service care planning and agreement negotiations;
  • Provide program discharge planning to dis-enrolled participants to ensure that the participants needs were met;
  • Act as an advocate for participants with outsourced service providers and others, as needed.
  • Conducts weekly home visits to participants residence;
  • Develop, prepare, facilitate and present training and educational programs and workshops;

Minimum qualifications include an Associate Degree in Business Administration, Public Administration, Sociology, Social Work or specialized training related to case management; 2-3 years of experience in case management; the ability to multitask and deal with a high volume of contact with the general public; and the knowledge of federal Housing and Urban Development Guidelines relating to homelessness. 

The minimum starting salary for this position is $49,516.30 plus excellent benefits. Applications will be accepted through August 18, 2026, at 5:00 p.m.
